Adhesive on the back of each heat sink. Apply one to each designated spot.

I just got mine. It’s indeed 3B+. It’s revision is a020d3.

Yes, there are two rectangular holes, one for GPIO cable next to the connector on the pi and one smaller on the short side opposite to USB and Ethernet connection. I think it can be used for display or camera
