What is the inside made of?

The inside and outside are one of the same------what you see on the outside is what you see on the inside. Depending on the number of pills and the type, it may rattle in your pocket a LITTLE. If we're going out for supper on a Sat. nite, I just set them in console and then in pocket when going into restaurant. Plus I like the picture of the cross------the daily heart pills-----tomorrow is not a given for people with heart problems

Can this hold a rosary?

A rosary with very small beads will probably fit but I am not 100% sure.

There are 4 sizes in that Pyx I don't know which one you are looking at. Sz1. holds 6-9 host Sz2. holds 12-15. Sz3 holds 20-25 and Sz 4 holds 40-50 hosts.

how many hosts does this hold?

It is a small pix. I think it would hold about four hosts. I only used it to bring Communion to one or two people.
