Does it work on 180mm rotors

It is not rated for use with 180mm rotors. That said I am using them with a 180mm front and it has worked fine. You assume the risk of failure running 180mm rotors

Will a max 210mm adapter work fine with a 160mm rotor? I ask b/c I have a 160mm, but seems better to have the more adaptable size if both work as well

You need a 160mm post-mount adapter for the 160mm rotor.

What is the difference between the one for 12mm axle vs 15mm axle. Is the axle measurement the thru-axle ?

Yes the thru axle max diameter is the difference.
