Does it have a device to lock the arm perpendicular to the wall or whatever it is mounted on?

No it doesn't. The arm swings on a horizontal plane only and sits on clamps which mirror the vertical and horizontal aspects of the pole.It does not swing upwards for locking nor can it lock on the horizontal.

What are the dimensions of the rectangular plates that touch the wall?

Rectangular at wall 125x75 (mm); Thickness approx 5mm. Plate is L shaped.

Is the illustrated pole with wall brackets included with the arm?

Yes, you have two wall brackets, one at the top and one at the bottom, they are part of the upright. I fitted mine on a brick wall, with a hoist, so i could raise my shopping and other bits to my house which is above my garage and as i have arthritis in my knees it save me carring thing up my steps.
