How do you adjust both the closing and latching?

If I remember rightly. You open the gate with hinge installed 45 degrees and the you will see a plastic end cap with a hole in it. Insert screwdriver and adjust tension accordingly by turning either clockwise or anti-clockwise for the latch push. Open the gate fully and adjust the same way for the swing.

is this suitable for pivot hinged gates

Not quite sure what you mean by 'pivot hinges' but as long as you get the axis of the gate hinges and the pivot (ie 'hinge') of the closer in line or very nearly in line then it should work ok. Hope you understand what I'm getting at.

How far do you have to screw in to position B to get the "hold open" to work?

On mine, the instructions about how to place the fixings were incorrect. I believe I provided this info in my review.

Do you only need one of these per gate?

On small and light gates one is fine but for larger you need to fit two. My wooden gate 1.8m x0.8M needed two.
