The neck massager comes detached and is optional to use. As pictured, it can be velcroed at any height and can be quickly and easily adjusted. The heat is not high but is pleasantly warm. The massage can target different areas of the back and offers different speeds. After only 5-10 minutes, my back and neck feel completely relaxed. Although pictured on my couch, to properly use the neck massager it needs to be against a taller chair or wall to work effectively. I decided to spend a little more and got this massager which came with a detachable neck massager. This really hits the right area for me. Pain isn't always evenly distributed. With the spot massaging feature, I'm able to lock the rotating nodes onto the lower lumbar region for continuous pressure. Oh what a relief. As a bonus I use the detachable neck massager independently for my lower back while on my recliner and sometimes in bed. For the heat function, I used it briefly and it provided additional relief but am looking forward to start using it more in the coming cooler months.
