Small/Medium size should fit fine for a 34 size waist.

how much absorbency is provided?

If you compare the maximum absorbancy underwear with a maximum absorbancy Guard, I looks like the underwear will hold Two to three times as much liquid. The underwear fit is much more likely to fit securely as well. If all you have is a little incontinence, you might be satisfied with the guard but for me, I am willing to pay more for the superior absorbent underwear.
