Are there measuring marks on the pitcher? Inside and out?

No, but it's a small pitcher and easy to eyeball the amount. I find it perfect for steaming milk.

Does this come with a spoon?

Hello, this frothing pitcher does not come with a spoon. Thank you.

Does the handle get hot when you're frothing milk

No. I have never had the handle get hot when using it.
