Although they are meant to be worn by themselves, are they comfortable to wear in a shoe?

Yes they are comfortable in a shoe. I wear them and slip on shoes to go to Pilates. Your foot will tell you which shoe fits the best with the product.

My shoe size is between 7.5 - 8. Should I get size small or medium for best fit? Thanks!

I wear a true 7M and I bought a small. They are pretty stretchy like a sock. Personally, I would go larger but I don't like tight fitting things on my feet.
