What tool do you use to tighten the retaining nut?

the tool is colloquially known as a "spanner" which in U.K. English is a general term for " wrench " I found such on the web but thirty bux was more than I would spend so I made my own, 15mm I.D tubing (pref steel) and file a pair of drive lugs, no more than hand tight or possible stripped threads may be on the menu

Are these insulated? Basically is the inside where the banana plug connects electrically connected to the threads on the outside?

No the OD threads are not connected to the inside where the banana plug connects. Yes they are insulated. There is a plastic jacket between the OD threads and the ID where the banana jack plugs into. I have purchased 500 of these in the past month. They work very well and good quality. They are a little difficult to solder. I ended up adding flux (plumbing flux) to the post to make soldering much easier for connecting wire. Even using flux core solder.

What is the thread on the barrel of these?

According to my measurements, the thread is M8 x .5. It is a very fine pitch thread.

Will these fit test leads for multimeters snugly or cess banana pin plug open screw type cable connector - tip banana plugs without falling out?

Yes, they fit multi tester 4mm banana plugs snugly.
