Can be right at top.
Quite tight at the top of my arm. I’m size:14 , 65 kg I snipped with scissors, bit like a pair of tights. Looks tatty now, but covers my PICC line and keeps it clean. At least I have 6 to play about with.

Wow amazingly surprised! Glad I didn’t go and spend a fortune on buying one sleeve, but spend $14 on six sleeves. It’s very lightweight and silky. Feeling not hot at all nice and long, but you can bunch it up enough to make it shorter if you want. Very comfortable does not cut off your circulation. Definitely protects. My pick line. Seems stable and adorable and definitely worth the money. Don’t spend your money on those expensive ones. This is definitely a go to sleeve.
