what kind of inhaler do use with the areokat?

It can be used with any inhaler. Inhalers for cats have the same sized opening as for humans and the common opening fits into the Aerokat.

releasing med: when hand over chamber then put mask on cat, or when mask on cat?

I take the end cap off the inhaler.  I shake inhaler well for about 15 seconds.   Then I insert the inhaler (fits perfectly into the back of aerokat chamber).   I keep the inhaler pressed tightly into the chamber by keeping my palm pressed against the back of it.  Then I depress (1) puff of the medication directly into the chamber (it will stay in the chamber a few seconds and the chamber will slightly cloud up).  Then I immediately apply the aerokat with mask attached to my cats face and the flap will start moving back and forth.  After 7-10 breaths, it's done and I remove the mask.  I find this way works better because my cat is much more calmer and accepts the treatment better than if I depress the inhaler after putting the mask on my cats face.   I think the sound  it makes when you depress it scares the cat and he tenses up.

Anyone know where i can get an "unmedicated" inhaler to use as practice and get my cat used to the air sound before actually using the medicine?

I don’t really see why you would need an actual inhaler to practice putting the mask on your fuzzy little friend. You can practice putting the mask over its face and just make sure it breathes between seven and 10 times using the visual aid.

Does this aerosol chamber inhaler come with an additional larger mask or only one small mask?

It comes with a small kitten sized one and a larger one size fits most 7-15 pound (avg sized) cats.
