Yes, its the best cup. It will not leak

Does the insert have to be placed a certain way?

There is only one way to seat the insert or the cap will not close. Very easy to press the round part, over the round hole in the lid.

How does it work? I just screwed the lid on all the way and it seemed like ny toddler couldnt get anything out and got frustrated.

Try removing the valve. However it can get messy because there will be no control once your toddler turns the cup up for drinking.
