Do the balls roll or are they fixed in place?

the balls roll (very smoothly) and are spaced at a comfortable distance apart for feet and back. the finish is very smooth. r.l.

What exactly can you effectively massage with this?

I would say outside/lateral sides of calves of your legs. It requires a lot of pressure. I bought it for the back of my shoulders but it hurt more. Better to get a massage. I threw mine away.

Is there a smaller roller for face?

This one may be small enough, depending on what parts of your face you want to massage. It's not big enough to get both sides of my back at once. In general, though, your own hands are usually pretty good for facial massage. These kinds of implements are intended more for parts that are hard to reach/apply pressure to.
