what is the cut out size in the door/panel for this?

Cann't give you a measured dimension as I used one half of the grill as a template for the cutting. But you have about 4mm all round so the cut out is about 452 X 127mm.

What thickness of door will his fit

Can't exactly recall the door thickness' this will fit, but sure it does fit 35 and 44mm doors. I fitted mine in a 35mm door and seem to recall it could have fitted in a 44mm one.

Is this vent suitable to provide adequate airflow for a small server room? The door is always closed and it gets quite hot in there.

While this vent would allow plenty of through ventilation, you will still need to fit an extractor fan or an other external vent to ensure there good air movement. Especially if you are using it in a coms room with a server. This would apply no matter what vent you fit in the door and quite honestly you won't do better than this one.

Can this vent be painted?

Probably not, unless you can get an acrylic primer for plastic or it will just flake off.
