Will this work on an EWI 5000?

Im not 100 percent sure but the EWI 4000 and the EWI 5000 are identical, except for the outer color. The mouth peice look similar on both models.

Can this monthpiece fit with EMI USB wind model?

The Akai website (akaipro.com) shows this as a replacement for both the EWI 4000 and the EWI USB.

This includes the mouthpiece cap? If not, how to get it?

mouthpiece cap not include. only same picture.

What sort of problems have players experienced with the original mouthpiece that now lead them to replace it?

There was no problem, only bought it as a spare in case I broke the original when trying to clean it, which I have still not tried to do.
