From wich plant is this d mannose?

Absolutely no idea. However it does work.

where is the product made?

Says to visit www.nowfoods.com/2810, which I will now do. Thanks for pointing this out, I am usually looking myself to insure it is not made in someone's garage.

pictured is Organic version but the ad description doesn’t state this. is it organic?

We've since switched to a different brand of powder, so we no longer have the bottle to confirm. Now we're using MicroIngredients powder, which is clearly labeled as organic. It's also less expensive.

What are the ingredients?

only D-mannose. I actually love this product much more http://www.amazon.com/Revolution-Cranberry-Concentrate-Fortified-Probiotics/dp/B00HNZMKAI/ref=sr_1_3_a_it?ie=UTF8&qid=1460644845&sr=8-3&keywords=urinary+tract+infection in addition to the D-mannose it contains a bunch of other ingredients to successfully get rid of UTI's. It has literally saved my life!
