should I put the mattress pad over or under this pad?

does anybody use this topper upsidedown? so that the flat side is up and the egg carton divots face down?

In a hospital setting. The flat side is on the bottom. Allows air to circulate. Would be interesting to hear from the manufacturer says. I’ve had one of these in the past. I out a thin mattress pad down on the bare mattress, then this egg crate topper, then a heavy mattress pad, then the sheets. It was wonderful.

I have a xl twin. will the twin fit?

it should be fine, you will have a few inches at the head or feet with no topper, shouldn't be a problem. you only need it under your body . these things are pretty versitile, i have a twin bed and ordered a king, folded it in half, and put it inside a twin mattress protector to hold it all together. i had to cut a little of the lenghth but it is easy to cut with shears. i put the part i cut off in a pillowcase for my dog.
