Can back cushions be purchased separately?

I don't think so. I did a lot of research before I ordered and I did not see where you could order only the bottom or the back. (I could have missed it though!)

Is the cushion that goes at the back 16 inches tall or 19 inches tall by itself? Looks like tha back cusion is smaller than the seat cushion.

The back cushion is smaller but it sits on top of the seat cushion so it's plenty long enough. The back cushions are also a little thinner than the seat cushions. But almost all your weight is on the seat cushion.

How thick are the cushions?

4 inches. Very comfortable to sit on

Can I order one additional cushion set

I needed 5 but it would only let me order 3 at once so I had to place a second order for 2.
