I am not the seller, but I believe that all ingredients are listed. The smell is quite stron but it's not an unpleasant menthol smell. I really enjousing this balm and gave it 5 stars as it is great pain relief and really helps me with my fibromyalgia

Can you use it on your forehead and head?

I would say NO as it says it is for Joint and muscle head i would not class that as this - its slightly heated when applied so i think it would be too sensitive to use on the head but it is up to you but i would strongly avoid. - left arms etc its FABULOUS

Has anyone used this gel for pelvic girdle pain, i.e hip issues?

Yes and it is very easy to use and a little goes a long way and works well for us on hip and lower back.

Would this help with rotator cuff issues ( pain , stiffness, loss of range of motion )

Yes!!!!Thats my problem too.A massage at night freezes pain so I can drop off to sleep.Stains pjs a bit but washes out.
