Would the large fit a 42 waist or would i need an xl?

Am not sure about that. I am 34 waist and 6ft tall...and I had a medium size suit and trousers. This fitted well.

Does it come with white fluffy arm and neck things?

Yes...very short trousers I found

Does it come with a wig?

No I don't think so. There are options on amazon to buy wig glasses and teeth separately if you check. It does I believe come with the shirt front frills and cuffs tied inside the suit which works well and you don't get so hot..
