How secure is this on a belt? Keys fall out ever?

Haven't had a problem dropping keys. I have one of these for each (4) set of car keys.

My key chain belt loop broke can i get a replacement part?

Try asking Amazon customer service. If you bought it recently, check your orders in your account as there may also be an option to return or replace it.

Will this scratch designer leather belts?

I don’t believe so. There are no sharp edges and titanium is not corrosive like aluminum or copper. I have no marks from this on my leather belts. If your designer belt is a coated/laminated faux leather or vinyl, I’m not sure if it would scratch, but I doubt it.
