How do I know if it is working

where is it manufactured?

This unit is made in China. It appears to be well made and does what it is designed for. The only fault I can see is that it is designed for snap on electrodes what limits its usage as pin types electrodes are plentiful and reasonably priced. The manufacturer could easily supply suitable lead wires so it could be used for both types of electrodes. In my case I had to reconfigure the snap on connector with a pin type connector using some small diameter brazing rod along with as soldering iron. I would think that supplying this unit with both type of connectors what would not cost much would very much increase its appeal as well as sales.

Anyone have problems with the intensity buttons not working?

You need to place the pads first, otherwise, intensity won't increase for safety reasons.
