Is is it a onsie or two piece?

It is a one piece bodysuit with detachable wings.

Does it fit true to size my grandson is 6 months but wears a 9 month

My grandson is 15 months and he's a chunck and his fit perfect. We ordered to size and it was perfect. Hopefully that helps.
