Is this shirt a slim fit?

I’m 5’7” and about 211Ib hefty muscular dude and I ordered an XL. The arms are tight but the mid section is quite baggy. It is also longer than your average shirt.... which I like. Pretty good shirt.

Does it shrink too much when wahsed and dried?

Yes please get another brand. After one wash in cold water the shirt shrunk significantly !

where is this t shirt made?

I am currently traveling for work, so don't have the shirt in front of me to answer this question, but I can tell you I it extremely comfortable, and would buy it again.
