Inside was a clear vacuumed-packed bag with the mattress pad inside. Then there was a Serta Label inside a clear plastic sleeve placed separately in the white bag. The item I received was defective with a hole big enough to insert a football in the skirting. I suspect it was returned & re-packed but it was sold to me as new. **Q: Will this work for a 12-inch mattress?** A: my mattress is 10 in and it fit perfect, so might be a gamble for 12 in **Q: Can you use this to cover a 3 inch mattress topper?** A: Hi Amy - not sure if you mean the 3inch foam in addition to mattress? We used this on a standard size mattress and it fit just fine. My guess is that it would be fine with a topper as well as long as mattress is not super thick.
