How do i know which way the retreive is set?

Our reel has been set at the factory for left-hand retrieve. The package comes with instructions to convert your reel to right-hand retrieve.

Can the 7/8 reel be used for red drum fishing? How much backing does it hold?

Yes it could be used for fishing red drums. I'm not sure how much backing, but a quick estimate would be 150-200 yards. It's a great reel for the money

Can i buy a extra spool for this item in 3-4wt?

Not sure I bought two reals. Easier to switch reals than spools
