Are you supposed to poke a hole in the tube to open it?

Take the cap off, turn the cap around and use the pointy part to poke a hole in the end of the tube. You only need a very small amount. Replace cap when finished.

The pump is more expensive. Is it hard to get all of it out of the tube that seems like a better value?

The tube is far better than the pump! The pump is so wasteful, as there is no way to just use a little bit. One pump worth is far more than you need. The tube lasts at least 10 times as long, as you only need a tiny amount with each application. This stuff sure works though!

This product has docosanol 10% and doco shield has the same amount, but is a lot cheaper. is there any advantage to abreva?

Really don't know never used coco shield. All I know is abreva works as advertised.
