I am not sure what the long term side effects are. The warnings say to consult a a doctor if you have diabetes as it is a sugar and can raise blood sugar levels. I am a borderline diabetec and have tested my blood sugar one hour after use and it seemed to have no impact. I don't know how it will affect others or people with full blown diabetes. I am not willing to try any other brand of d-ribose because this one works so well for me. I dont take it after about 2pm as that is what the Dr. who originally sold it to me told me as it may affect your sleep if you take it later. I take it first thing upon rising and then another teaspoon mid morning if possible or at least before 2. I have insomnia so am not willing to risk taking it any later though I am not sure it would impact my sleep if I took it later. I always take it before and after i exercise in the morning. I just can't say enough about this product. It seems to good too good to be true but it is true, at least for me.
