Hello, we do not sell pillowcases separately, but the pillowcases of this pillow are washable.

What angle should neck be at when sleeping on back - should chin be pointing up rather than downward or flat?

I find this cervical neck roll is great for me , sitting in the lounge, it’s great for the car by positioning it in the right area for me to have comfort. I haven’t tried it in bed but you as the person using it must find the position that brings you comfort.

Can you use this support on top of other pillows .or as it to be used on its own.

I am editing my previous reply as I have ordered another neck product - the one u r asking about ix v good for under neck and yes use on top of pillow it's memory foam has its own cover that can b used for washing. I have severe osteoarthritis in my cervical spine and I find this really good if u want to read or use tablet in bed.

Would it squish so it can be packed in a carry on suitcase?

It won't go flat as its memory foam but when it was delivered it was packed / pushed in half.
