Can this foot protective gear be washed in a washing machine?

Yes, but would recommend to line dry. DO NOT put in dryer. We used Oxy clean as a stain remover/prewash then washed on cold, just to prevent shrinkage.

Are size medium and large for adult?

Pay attention to child large and just Large. But an adult probably large to X-large for normal sized people that are not obese.

Is there a size guide?

No guide, under 8 yr old a small, 10 yr med. if kid over weight go up a size

What size for a 4 year old ?

Measuring our daughter's shin length before searching helped us. Looking for brands that offered a size chart also helped. She was pre-teen when we purchased, so I'm not sure if I would be best to answer on a size for your 4 year old. I know it's hard to find the right fit and choice. Before you purchase, try to look to see if they do offer returns, that way if they don't fit you can get a refund on your return. I hope this helps, wish it could be more.
