Can you put this on a 4x4

You sure can. Rather than using both of the included brackets to clamp onto a 2" EMT conduit, you can use one of the brackets and run wood screws through the 4 bolt holes and into the 4x4 post.

Question 2

Should these be painted white? To keep the box cool here in Florida? Seems the entrance would stand out better if the box were white?

It is interesting that your asking about white color boxes.  In the past I had visited with a guy that raised wood ducks for a living.  He told me he had a white box in his pen that the hens would not use year after year so he decided to paint the box green the same color as my boxes.  He said the hens started using right away.  I know this is not scientific, but this how I chose my color.  As for the heat I do have testing to prove my boxes stay cooler than mental canisters or traditional wooden.  The reason is the vents that were designed into the box.  Another feature that was designed in is the opening is a little bit larger then standard for wood ducks to accommodate the Black Bellied Whistler that you have in your state.  If you visit my Facebook site (DuckHut) you can see three pictures of a drake wood duck standing on top and a hen coming out of the box.  These pic's were taken in Fl. Can this be used for other kinds of ducks?

If I’m not mistaken there are seven different types of cavity nesting ducks.  That could use, however if your raising domestic ducks I believe there have been some people that are using Duck Hut for that. Kent

Are there supposed to be screw thread inserts in all 4 holes of both brackets? The brackets I received only have threads in 2 screw holes..

No each bracket has two inserts.  Both brackets are exactly the same.  When you turn them back to back two cap screws  go into each bracket.   That's  all true providing both brackets each have two inserts.
