How do you get it to spray the first time? It won't push down.

There is a place next to the apparatus that you push down. Point the nozzle at an angle towards opening of one side of your nose.

Does it help with mucus?

Depends on how severe your mucus is. It's basically just keeping your nostrils from being dry. You can try it but it didn't work for me.

is the cap sealed with wrapping all around top, got one with no wrapping?

Mine came in a box and I don’t think the cap was sealed. Just the nozzle was with a breakable little seal.
