No, it does not have a knife inside it. But it has a significant puncture point on the top of the pen.

Q: How do you retract the pen so that it doesnt write all over your clothes?

You have to twist Tue pen in the middle about 6 tim es and the cart rage will retract if you twist about 15 times that's how you change the cartrage
