Will this work to convert W520 to W540?

NO. Not sure where the above people are getting their info - I have a 540, and borrowed someone's 520 PSU, and it didn't fit - this adapter fits the *W510* barrel, which doesn't have the 2 little "wings" inside the barrel. The 520 PSU *does* have the wings (2 ridges on the inside of the sleeve), so will not fit into this adapter. Meanwhile, the W510 PSU barrel fits, but it's only a 135w PSU, and the W540 will not recognize it (they use resistance to recognize the different wattage chargers).

Will this convert my 90w 20v p/n 44t4426 to fit my think pad t450?

I had T420 with the round connector and this converted it to the rectangular connector for the T450. It fits snugly but not too tight to the old cable and works great. I would highly recommend it.

which tip size do I want, to go from x220 to thinkpad yoga?

This is an excellent converter for the new form factor of the Yoga power cord. If your x220 uses the power adapter with the round yellow tip and pin the middle, this is an excellent way to save jingle from having to buy an additional adapter. I have had no problems whatsoever and use two of my old Lenovo cords (one for travel and one at home).
