Instructions on bottle day wash out after 40 minutes, after straightening, but I've read lots of places to leave in for 3 days. What's correct?

Here are the complete instructions for use: 
HOW TO APPLY:
1. Wash the hair twice with Clarifying Shampoo, rinse and towel dry.
2. Blow-dry the hair on a medium heat until completely dry.
3. Comb the hair thoroughly.
4. Shake the product well before use.
5. Brush the Brazilian Keratin Treatment onto sections of hair (avoiding
the scalp) and comb out the excess.
6. After 10 minutes, blow-dry the hair on a medium heat until
completely dry.
7. Using hair straighteners set at 220°C or 430°F, straighten the hair
up to 5 times to seal in the treatment.

8. Do not wash or tie up the hair for 3 days. If hair gets wet re-
straighten with hot hair straighteners.

9. After 3 days, wash the hair with Salt-Free Shampoo and
Conditioner.
IMPORTANT: Please follow these steps without taking any shortcuts.

How many applications per bottle? On shoulder length hair

It really depends on the density and length of your hair, as well as on how damaged it is. Some hairs absorb more keratin than others. One bottle should be enough for at least 1 treatment, but there is also a possibility that there will be some keratin left over in the bottle as well.

After applying do you need to blow dry or use a hair straightener?

You are supposed to blow dry it and then use straighteners. My hair is thin and it caused breakage so my hairdresser  just blow dried it straight without the straighteners but in the end I had to give up using it. If you have thick hair it is probably ok.
