Has the mill scale already been removed or I still have to soak it in distilled vinegar to get rid of it?

Hi, No these are made using quality pickled and oiled material so just need a good wash and then a season

Can this fully cook a pizza (top & bottom) in a wood fired pizza oven without burning the bottom - not Ooni

As long as the oven is hot enough to cook the top of the pizza this can be used in a pizza oven.

Can you use it on a gas BBQ?

Yes its possible to use on a gas bbq, you may just need to re-season more often.
