The size relates to the diameter of the head size. The medium is popular one because it is a standard size. The larger one which is 3" and has a bigger head. Unlike with a round brush, the square shape allows you to pick up sections of hair using the corners of the brush without pulling the strands all the way around the brush, which has always caused tangling for me with a round brush. This brush is easy to maneuver while blow drying, and the lift and shine that results is ideal.
