Am I supposed to hear a sound when I shake my wheel a bunch of them have loose looking ring in the middle what is that?

The silver ring between the bearings when you look through is called a spacer. It prevents the wheels from being over tightened. Without them, you could tighten your skates until the wheels don't roll. With them, you can't do that. Prevents people from getting hurt

Do I need to purchase bearings separately?

These come with bearings, and they're not bad. Eventually, you'd want to swap out for Rollerbones or other high-quality bearings, but to start out with, the stock Tobwolfs aren't bad. (They were a darned sight better than the bearings that came with my brand new Chicago skates, LOL!)

Will these wheels fit vanilla skates?

They should. You just need the right size bearings. And today’s roller skates take 8mm size bearings.
