Q: Am I supposed to hear a sound when I shake my wheel a bunch of them have loose looking ring in the middle what is that?

The silver ring between the bearings when you look through is called a spacer. It prevents the wheels from being over tightened. Without them, you could tighten your skates until the wheels don't roll. With them, you can't do that. Prevents people from getting hurt

Q: Do I need to purchase bearings separately?

These come with bearings, and they're not bad. Eventually, you'd want to swap out for Rollerbones or other high-quality bearings, but to start out with, the stock Tobwolfs aren't bad. (They were a darned sight better than the bearings that came with my brand new Chicago skates, LOL!)

Q: Will these wheels fit vanilla skates?

They should. You just need the right size bearings. And today’s roller skates take 8mm size bearings.

Q: Hi-I am buying skates with 54mm wheels; can I replace them with 58mm wheels? (For a 9 year old)

Yes you just need the right size bearings. Roller skates would be 8mm size.
