Is this unscented?

No scent. I’m very allergic to scented products and this doesn’t bother me. Also, FYI it really works. Takes about a week and then you begin to notice that it’s helping.

Why canton  you use this on your lower back or shoulders?

Different government entities have different standards and/or tests they rely on. If the European Union, Australia and New Zealand have approved a medication which our own FDA has not, don't assume our scientists are the only experts in the world. 
I've been using Diclofenac Gel for the last two years on my lower back, to the side of my last vertebrae (which had a hairline fracture), and down my right butt. I use LESS in these three sites than is allowed for one lower joint.
MOST IMPORTANTLY, my general practitioner is an 'Internal Medicine' doctor who prescribes this Rx for me (I only recently discovered it was available OTC); I've also been in the care of an 'Orthopedic practice' who verified my slight fracture would have to heal on its own, and a recent X-ray so confirms it; I'm also under the care of 'Pain Management' specialists (I live with "Refractory or Intractable Chronic Daily Migraines," and have for 27 years and counting) who confirmed they recommend 'Diclofenac Gel' for muscle pain such as I have. I also use less conventional medical therapists, including a renowned 'Chiropractor' and a 'Massage Physical Therapist' who specializes in the musculoskeletal systems of our bodies.
And do heed the warnings regarding duplicate use of NSAIDs, along with the potential impact of NSAIDs on certain organs. Which means regular check-ups, physicals, blood work, and whatever else your GP and specialists may recommend. 
You do all that, and I suggest you can safely use 'Diclofenac Gel' for muscle pain and/or joint pain WITH CARE. Wishing you less painful days!!
