I don't know if you need to be sharpened before using the knife, but the knife will benefit anytime it is sharpened.

Is the handle comfortable for large hands?

Yes. My hands are small and find the knife very comfortable to use. I believe large hands will find it even more comfortable as the handle has an adequate girth. It’s reasonably sharp as well and I am able to cut my fruits perfectly.
