I use it for neck pain and stress relief. It has helped to reduce that very well. I use it when needed and usually I do that daily for some time each day to relieve the stress. I have not tried to sleep in it yet but I might depending on my stress level - if I do I will update this review if there is anything more to contribute. The pads are comfortable and the adjustment knob does work and is easy to use. Note I adjusted it while not wearing it as that makes the knob easy to turn without any pressure on it. The 2 velcro straps one on each side are very easy to apply and remove plus you can really get the right amount of tension on the back-front parts of the brace. The velcro is very secure, so you can leave one side of the collar affixed to the velcro while putting the collar around your neck and affixing the other side of the velcro. BTW, $40 is a very good price. I purchased my collar through medical supplier recommended by my doctor and approved by my insurance. The cost was $100. I purchased an additional collar on Amazon to send to my sister's house (so I would have one there during my stays). I paid $80 for the one I purchased on Amazon. So $40 is a great price.
