How did it put the tongue at the roof of the mouth

I had to practice.  I put the trainer in my  mouth, just position my tongue on the roof of my mouth, and then (here's where it's hard to explain) I just kind of suck in my cheeks and it creates a vacuum.  Then my tongue stays up until I open my mouth or move my tongue around too much.  I guess that breaks the vacuum.  That seems to be the trick, making a vacuum in your mouth.  That's the best I can describe it.  Sometimes when I roll over in bed, the little front tab must get jostled and I lose the suction feeling and just do it again.  Good luck!  I really like mine.

Question 2

I use the nasal pillow mask for sleep apnea. Have tried Somnifix strips to keep my mouth shut but they didn't work. Will this product work?

I've been using a competing product with almost identical name, "nosebreathe", together with a nasal mask for more than 15 years; works perfectly for me. Unfortunately there is only a single supplier, a dentist, who has done almost nothing to market it; Also, it is quite pricey.  That said, if you carefully take care of it, it can last for 18 months or more. I've stocked up on a bunch in case provider every retires.

One review says doesn't stop mouth breathing with cpap. i can't tell from the product description if it is even intended to help cpap users. anyone?

It took some time for me to get use to it. I was a mouth breathing at night. 
I would recommend a book for you also called “The Oxygen Advantage “. There are breathing exercises that do work. 
Thinking that this mouth piece by itself will help from the get go without a break in period is hard work to stop a back habit of mouth breathing.

Does it matter if you wear it with the notch up or down?
