Should I use bare skin or with clothing is it more effective with bare skin?

I started with a very thin t-shirt, even at my neck...that seemed to help but not as much as I had wanted...So then after about a week, I tried it on my neck bare skinned and I felt like it worked better. Another 2 days later I tried it on bare skin on my back too, and it's wonderful that way. Hope this helps.

Can you remove the foam and wash the covers of the mat and pillow?

I just throw mine in the tub, both mat and foam together along with some liquid body soap and let it wash itself. Then I let it air dry on the side of the tub.

Is the cover removable?

Yes, the cover is removable. There are two velco strips at one end. You can take the foam strip out and clean it too if you want. I have had the mat for two years and have never removed the cover.

Can the pillow be used for your lumbar section as well?

Yes. I use it for lumbar more than my neck. I tend to use the "pillow" by itself for this use.
