Question 1

A reviewer complained that this is actually a Fluidmaster 400A.  Is that what you got or was that reviewer ripped off?

It's a variation on the 400 series.  Fluidmaster makes maybe over a dozen different versions including OEM versions sold by Kohler, PerforMAX versions, ones with brass shanks, etc.  The only thing that seems to be exactly the same among all versions is the float and the (replaceable) seal.  Almost every Fluidmaster fill valve (including the 747PRO) uses the Model 242 seal.  As others noted, it comes with a set of restrictor inserts to reduce the amount of water going into the bowl.  You want to use the one that just manages to fill the bowl to the maximum around the same time as the tank is filled.  I found the green one is installed in 400A valves in an American Standard Cadet 3.  Those factory installed valves are EXACTLY the same as retail 400A valves, except for the insert and that they use a hex nut (which requires a tool to install/remove) rather than a wing nut that has to be tightened/loosened by hand.

Someone mentioned the roller clamp in the PerforMAX.  Fluidmaster has a newer one (400H) that uses a dial that varies the restriction.  Much easier to adjust than the roller clamp.  Fluidmaster has also been adding a bigger adjustment knob to most of its fill valves, so the photo might not reflect that yet.

Question 2

I purchased a Kohler Fill Valve that requires you to adjust the length of the stem by turning one part in or out...Mine will not go smaller or larger?

Mine had the same problem! It just spun. Somethign was stuck and I got extremely frustrated. The directions said to turn, but that did nothing. Turns out the whole base can be pulled straight out if the retaining collar is slid upward. After sliding that collar up, give it a good yank. Just don't break the valve and float at the top; I just about did. This fluidmaster video explains how. It's not the exact match, but is pretty much the same exact setup since Fluidmaster makes the official Kohler fill valve. Question 3

This says universal.  Can it replace the old ball and arm style fills valves?

Well my Kohler toilet had a Ball and arm that came in it originally and it started leaking, and I used this to replace it. Was really quite easy to do, most difficult part is removing the water inside the tank that's left over after you turn off the water and flush. I used several old towels. You could also if you had enough space undo the fill valve and have a container to catch the water. Other than that its quite easy.

Question 4

Does the Washer, Locknut and Clip come on this like the original Kohler part this replaced OR is it no longer needed with the new replacement?

All required washer locknut and clip come inside the package- yes you still need them as it will install as an exact match to what was there previously
