I am looking for a men's crotch antipersperient spray that lets you tip-over the can.

I use medicated talc for best crotch scent results.. (eg gold bond). I definitely wouldn’t use an aerosol on my genitals

Will this work for a person that is unable to lift their arms high after surgery such as rotator cuff?

It might work if you can raise your arms about halfway. You don't have to raise them very high. On the other hand, you might do better with a creme deodorant, which is easy to apply and doesn't require you to move your arms much at all.
