Does this come melted

No it is solid. You can run hot water over bottle and it liquifies. If the bottle is in a warmer area of home it will be liquid. It solidifies in cooler temperatures.

Has anyone used this on their eyelashes? Or any recommendations for using coconut oil on lashes?

I have no clue. But I use it for my hair mostly, so maybe it could work. It will also depend on what you what the product to do for your lashes coz I use it mostly as a moisturizer.
