What do you mean with transparent front ones? These bags are one piece, grey color, the glue in the barrier is not that good if the person has a bulge the barrier will lift. the glue is not strong enough. Besides that issue the bags have a pretty good performance.

Is this a reuseable item or one use and throw away?

Should last 3-4 days. It isn't really a question of 'one use' because it is constantly being used. It is drainable so it doesn't have to be replaced when full.

Can this be used with a belt

No, this pouch does not have the belt tabs needed for use with a belt.

Do these include the flex barrier with the pouches? Or do I need to order those separately?

These are a one piece system. They stick directly to the skin. You can use a flex barrier if you want, but it isn't necessary. But a flex barrier is NOT included. It does come little stickers to cover the aeration holes for going out
