Is this bottle designed to drink from directly (like a big mug), or to pour into a cup (like a small thermos)?

This is more of a big mug thermos. You can see in the photo that there's a button you push that pops the lid up so you can drink straight from the bottle. The only downside to this bottle is that it's so big that it doesn't fit into a cup holder, but for my job where I work 12 hours it's great, because it allows me to have easy access to hot tea for the entirety of my shift.btw. You can also twist off the lid, and there a 2 inch diameter opening so you can put ice in, or for pouring purposes.

Does not fit all the way in a 2018 Mazda CX-5 cupholder. Only goes part way in. Hit a bumb and it falls over.

is it leak proof

Some of you guys writing how it leaks a little or a lot must be chimpanzees. DON’T FILL TO THE VERY TOP, ONLY FILL BELOW THE THREADS!!! 
First off, it does not leak. I’ve tested it by shaking flipping upside down then open and unscrew the cap.  The most you get is a couple of drop and that’s only after shaking the lid!  Second thing, it keeps hot liquids nice and warm up to the 19th hour!  I tested this by heating up water nice and hot then left the thermos outside at 1pm temperature being 55 F.  Over night temps fell down to 36 degrees.  This morning checked the thermos at 8am (19 hours later).  Water was nice and warm after 19 hours.   You won’t go wrong buying the THERMOS!  Customer service is good as well.  I had a leakage problem with another of their 16 oz product.  This product was made for people with arthritis of which I was not aware when I bout it so each time I used with coffee and I would tip it over be accident while driving, coffee would leak out the ‘open and close’ clip while on the closed position.  It was designed that way in order to open easy for people with arthritis in their hands.  So the customer service rep understood my concern and was kind enough to send me a new lid that was designed to be leak proof and a little tighter to open and close the clip for people with no arthritis.  Worked well.  Bonus lid also has a tea bag hook!   Great American Made Thermos.  Buy with confidence.

does it fit in car cup holder

In my car (a 2013 Hyundai), it does not fit very well. It sort of just sits on top of it but won't fit all the way down into the cup holder. That is one (small) drawback.
