There are three padded straps with buckles on the outside of the splint to ensure immobilization and adjustable dorsi flexion straps for stretching of the plantar fascia and Achilles tendon. Recommend HCPC L4398Preparation1. Begin by un-attaching the 3 straps across the front of the night splint using the quick-snaps provided. Do not loosen the dorsi-flexion straps at this time. Application1. Place your foot into the night splint, making sure the heel fits snug against the heel of the splint.2. Attach the bottom strap and adjust until snug. The straps should be fed under the dorsi-flexion straps.3. Continue by attaching the remaining 2 straps as described above. Adjust straps for tightness as needed. Dorsi-Flexion Adjustment1. Un-attach the dorsi-flexion straps and simultaneously pull to adjust the dorsi-flexion to the desired level. Review: This is a legit night splint - not a sock - Very sturdy, effective night splint - in my case for Plantar Fasciitis and associated ankle strain. It is much more substantial than the other splints I have used (various toe to shin straps and bendable front of shin/ankle aluminum brace). It is a bit of work to get it adjusted, but once there it has three easy clips to take it on and off so easy thereafter. It is not a sock like many of these splints, but rather a hard plastic back of foot/ankle/shin support so it might be tough on sheets (I saw no issue, but was wary of this point). The included velcro wedge is really handy as it does push up the front of the foot about another 3/4 inch, keeping tension on the under-foot tendons, as it should. After just a few days I noticed significant improvement in my foot and was able to put it aside (until the next time). The only note for buyers I would suggest is that unlike so many things on desertcart, this item actually fits LARGER than you will expect. I have a size 11 foot, but found the largest size too large (I couldn't fully tighten the straps), so returned it and got the next size down - that works really well. When I first started wearing a splint it took a couple weeks to build up the tolerance to more than an hour. After a couple of weeks I was able to sleep through the night. Before switching to this splint my foot was warm and would cause me to wake up and I would remove the splint. I do not have that problem now and can sleep comfortably through the night with it on. **Q: Can you tell me if it is recommended that a women’s size 7.5 should purchase a small? The size small states less than 7.5, however a medium says 8-10.** A: At a shoe of 7.5 you must get a Medium. I have a shoe size of 6-7 depending on the shoe. In the small boot my toe touches the top.
